[TN] What form does a judge sign for a name change?

I filled an application for name change in Sevier County Tn. I have a docket number. The girls at the courthouse advised me I had to download another form with my case number and that is the form I will personally give to the judge. I have no idea where to find this form.

I filed an application for name change in Sevier County Tn. I have a docket number. The girls at the courthouse advised me I had to download another form with my case number and that is the form I will personally give to the judge. I have no idea where to find this form.

I understand the importance of finding the correct form for your name change application. Have you checked the Sevier County Tn website for downloadable forms related to name changes?

Have you tried reaching out to the courthouse or the clerk's office directly to inquire about the specific form you need for your case number?

They told me it is very hard to find..

Is there anything else the Lawyer should know before I connect you? Rest assured they'll be able to help with your form.

no

What you need here is likely the court order approving your name change, if that is what they say you need. Please see the attached for that order.

Jon

If the clerk mentioned that you need a form for the judge to sign, that is the court order. It is attached for you. Just bring it with you and hand it to the judge; this will grant your name change.

Thank you. I think this is exactly what I need. Have a great day.

Jon
