You and the notary must be in the same place when you sign. A Washington-commissioned notary must witness your signature in person (or by approved remote notarization, if using an online notary platform registered in WA).
Do not sign the form ahead of time. Bring your unsigned consent form and valid ID (like your driver’s license or passport). The notary must see you sign it before stamping.
The notary section (“acknowledgment”) usually looks like this:

On this ___ day of ________, 2025, before me, [Notary’s Name], a Notary Public in and for the State of Washington, personally appeared [Your Full Name], known to me (or proved to me on the basis of satisfactory evidence) to be the person whose name is subscribed to this instrument, and acknowledged that they executed the same for the purposes therein contained.

In witness whereof, I hereunto set my hand and official seal.

I thought that was the service you will provide. Where can I get a notary?

It would not be possible to notarize things online. But you can get a free notary from your bank. There are also mobile notaries who can travel to you, but they would charge.

Can any bank provide the notary service?

Are online notary also acceptable?

Usually online notarization is illegal, but Washington State allows online notarization. Remote Online Notarization (RON) has been legal there since March 2020.
