In Nevada, a Power of Attorney (POA) typically needs to be signed in front of a notary public. Since your mom has onset dementia, it's important that she fully understands what she's signing at the time of signing. If she can demonstrate understanding, her signature along with a notary's acknowledgment should suffice. (NV Rev Stat § 162A.870)
