Probably not HIPAA, but it may be an ADA problem. HIPAA generally applies to covered entities like health plans, health care clearinghouses, and certain health care providers that conduct standard electronic transactions. A regular convenience store is usually not a HIPAA-covered entity just because it asks you for paperwork. For a service dog in a business open to the public, the ADA rule (28 CFR § 36.302(c)(6)) is different. If it is not obvious what service the dog provides, staff may ask only two questions:

• Is the dog a service animal required because of a disability?
• What work or task has the dog been trained to perform?
