Since your mother is still competent and willing to step down as your father’s primary agent under the POA, she can execute a formal Revocation of Power of Attorney document. This should:
- Clearly state her intent to revoke her authority as your father’s primary agent (financial and medical).
- Reference the original POA documents by date and title.
- Be signed and ideally notarized for recognition.
Once signed, provide copies to:
- Financial institutions and healthcare providers.
- Backup agents named in the POA (including yourself).
- Keep a copy with the original POA documents.
If your mother is unable to sign later, guardianship proceedings may be necessary.

I need a form that will revoke my mother’s financial and medical power of attorney for my father. My father has Alzheimer’s, and my mom is his power of attorney. I was secondary. My mom’s health is declining, and I need to be able to take care of my dad if she is in the hospital.
