I'm finally ready for my son to get a legal name change. He was born abroad to two US citizens, but his Born Abroad birth certificate has a last name that's different than his social security card and state ID. I'm just trying to verify what forms I need to file for a legal name change so that he can get a driver's permit.

The only one I have so far is what I assume is the Petition for Name Change I filled out through the GA court website. I know that I need something to submit to a specific local newspaper as well to announce the intent to change his name.

Is that the only two forms I need to file? And which, if any, of them need to be notarized?

So the newspaper and is not filed with the court. It will run for four weeks, and the affidavit of publishing will come from the newspaper. You will then file that affidavit with the court. You will also submit a proposed order of name change.

And that's all there is to it?
