What do I do after completing and notarizing gender and name change forms?

Oh, is that what we do now? Take the forms to the Nassau County Clerk’s Office? We didn’t know what to do with all the forms now that they are ready to be submitted.

Do we need to submit this form as well: Order Granting Name Change and/or Sex Designation Change for Individual Adult?

You do. That will be signed by the judge to grant the order. Have you also done fingerprints?

We have this one as well: Name Change and/or Sex Designation Change Petition for Individual Adult.

That would be if the court requests it. The petition is the beginning form.

So far I have the following:
- Name Change and/or Sex Designation Change Petition for Individual Adult
- Notarized Affidavit of Gender Change for a Person 17 Years of Age or Older
- Application for Amendment of Certificate of Birth for Gender Designation for an Adult (for persons born in New York State, outside NYC)
Once this is all done, I need to get a new state-issued ID—but that’s self-explanatory I think. So what am I missing?

You also need the orders for the name and sex designation change. Do you have any criminal background?

No.

You will just need the petition and the order. You then submit the order to the DMV to update your license.

Okay, so to clarify: I need to submit all the forms I listed to the county clerk except the DMV items. Is that correct? Am I missing anything? The worst thing is going there and being told you have to come back. What about the Amendment of Certificate of Birth for Gender Designation?

You would submit the court order to Vital Records to amend the birth certificate.

The Order Granting Name Change and/or Sex Designation Change for an Individual Adult looks very complicated. I don’t understand the wording in the “optional provisions” part, so I don’t know what to check off or give good cause for.

You don’t have to use the optional portions—that would be if you had a criminal charge, for example.
