[WA] How can I get a document notarized if it doesn't have a space for a notary to sign?

The notary should be able to notarize your signature on a separate sheet of paper called a Notary Acknowledgement, on which they can indicate the name of the document and the facts that you presented an ID and they observed you sign the document (WA Rev Code § 42.45.130(1)). The Notary Acknowledgement is then stapled to the document.

I am trying to get a student visa signed for my daughter and we need paperwork notarized. The official Czeck paperwork does not have a space for the notary to sign. If we add the notary wording for Washington state, does that void the paperwork?

The notary public should have that form. I am very surprised they did not have it. You may have to go to a different notary.
