[GA] How do you change a child’s name with both parents’ consent and mismatched birth records?

I'm finally ready for my son to get a legal name change. He was born abroad to two U.S. citizens, but his Consular Report of Birth Abroad has a last name that’s different from his Social Security card and state ID. I’m trying to verify what forms I need to file for a legal name change so he can get a driver’s permit.

That’s the Notice of Name Change. It’s a limited form and only needs to state that you are filing a petition for a name change and that anyone who intends to object has 30 days to do so. (GA Code § 19-12-1 (h)(2))

The notice for the newspaper is not filed with the court. It will run for four weeks, and the newspaper will provide an affidavit of publication. You’ll then file that affidavit with the court. You’ll also need to submit a proposed order for the name change.
