[TX] How do I revoke multiple powers of attorney with a single revocation letter?

My sister-in-law has two medical powers of attorney that were made in November of 2024 and the other about 3 years ago. She now wants to revoke those and appoint a new agent. In a revocation letter should both agents (one for each of the two POAs) be listed in the same letter or can we make 2 separate revocation letters for her?

That’s what I thought but I'm not finding a form to accommodate two agents with different addresses.

I did see this one and started it but there is nothing to indicate that I can put both names and addresses on there ....they are two separate POAs.
