How to change your name and what government agencies to notify:

Once you have your certified marriage certificate, take it to your local Social Security office to update your name. Bring your current Social Security card, photo ID, and the certified marriage certificate. You will complete Form SS-5, which is available at the office or online. The updated card will arrive by mail within about two weeks, and your number will stay the same.

After that, update your name at your state’s DMV. Bring your new Social Security card, marriage certificate, and current driver’s license. Most states require you to do this in person. You can also update voter registration at the same time.

Next, contact your bank, employer, credit cards, and insurance providers. Provide a copy of your marriage certificate and your updated identification. If you have a passport, you can file Form DS-5504 (for a recent passport) or Form DS-82 if your passport is older.
