For Connecticut (and really most states), if you’re an adult and want to change the name on your birth certificate, you usually need a court order for a legal name change first. That court order can be from a court in the state where you live now (Ohio), not necessarily from Connecticut.

Once you have that order, you send a certified copy of the court order, the CT vital records form and fee to CT DPH or the Stamford town clerk. Then they amend your birth record to show your new legal name.

So the sequence is usually:

Step 1: Legal name change in court
Step 2: Use the court order to update SSA, DMV, passport, and then CT birth certificate

So your understanding of “petition name change, then change the birth certificate in CT” is basically right.

You need to decide what your official, permanent legal name should be:

  • Keep the longer name everywhere, then update Social Security, Ohio license, etc.
  • Keep the shorter name you actually use, then:
    • Do a court name change to that exact name
    • Update your CT birth certificate and future passport to match

From what you wrote, it sounds like keeping the shorter name is what you want, because that’s what your Social Security and life records use.

Since you live in Ohio, this is the usual path:

Step A – Ohio adult name change
Go to your local county probate court (in Delaware County, for example) website and look up “adult name change.” (OH Rev Code § 2717.02)

You’ll file a petition that says:

  • Current name (as on your CT birth certificate)
  • Requested new name
  • Place and date of birth (Stamford, CT; DOB)

The court will:

  • Set a hearing date
  • Often require some public notice (unless waived)
  • Then issue an order granting the name change if no problems

You’ll get certified copies of that name-change order. That order is your “golden ticket” for everything else.

Step B – Update your records (not necessarily in this order)
Once you have the court order:

  • Social Security – file Form SS-5
  • Ohio driver’s license – take the order + updated SS record or card
  • Passport – apply for a new passport in the new legal name
  • Connecticut birth certificate – send the court order and CT’s amendment form & fee to CT DPH / Stamford

I have recently found that the name on my birth certificate does not match the name on my Social Security record. My parents made the mistake. I was born in Stamford, Connecticut, but I currently live in Delaware, Ohio.

From what I understand from the Connecticut public health website, I need to petition my name change and then birth certificate name change in Connecticut. Is that right?

I understand how concerning it can be to have discrepancies in your legal documents. Have you already started the process of gathering the necessary documents for the name change petition?

I have not started the process, but I do believe I have the documents, such as my passport, Social Security card, and Ohio license.

Are you currently facing any deadlines or specific time constraints regarding this name change?

The only constraint I have is that this summer, June 2026, I will be going to Costa Rica.

Is there anything else the family lawyer should know before I connect you? Rest assured they'll be able to help with your name change.

No.

Hello. My name is Queeneth. I am an attorney with over 8 years of legal experience. I will do my best to assist you today. First, I need to ask you a few questions to give you the best answer. (Kindly note that our conversation does not establish an attorney-client relationship.)

Have you ever used the full birth certificate name on any official documents or records besides your birth certificate? Are you hoping to have all your documents—including your passport and Social Security card—match the corrected name after this process?

I did not set it up, but my parents used the full name on my passport. For everything else in my life, I have used a shorter version of my name.

My parents used the shorter version on my Social Security card.

Thank you for the feedback. Please allow me some time to prepare a response. I will get back to you shortly.

Hello, I was just wondering what you were able to determine.

If you need any more information from me, please let me know. Also, do you happen to have a time frame that you may need to develop a response?

Thank you for your patience. You’re reading the CT vital records site and think you need a court name change first, then a birth certificate amendment. That’s basically correct, but you might not need to rush it before your Costa Rica trip.

After that, all your “big” records should match.

You’re traveling in June 2026 and your passport currently has the longer name. Airlines and border control care about the passport name, not your SS name.

For travel, your airline tickets MUST match the name on your passport. If your passport still shows the longer name, book the ticket using that exact name. The mismatch between passport and Social Security isn’t a travel issue; it’s more of a tax/employment/ID consistency issue.

Because of that, you have two realistic options:

Option 1: Fix everything after your June 2026 trip

  • Use your existing passport to travel
  • Make sure your plane ticket matches the passport name exactly
  • After you return:
    • Do the Ohio name-change process
    • Update SS, license, then get a new passport
    • Then amend your CT birth certificate

Option 2: Fix everything before your trip (riskier)
You’d need time to:

  • Get a name-change order in Ohio
  • Update SSA
  • Apply for a new passport and get it back before June 2026

If any step is delayed (court backlog, passport processing delays), it could create stress before your trip.

That’s why I usually tell people in your situation: travel first, fix later if the trip is already planned and not far away.
