[OR] How do I remove my deceased spouse’s name from the deed for our home?

I am trying to have my deceased husband’s name removed from the deed for our home we owned together. I have filled out an affidavit of survivorship form and will sign it with a notary. I also have a certified death certificate. It will be filed in Washington County, Oregon. Do I need any other forms to have his name removed?

Reviewing the transfer-on-death deed I just completed online, do we need to include a spousal acknowledgment? Our intent is that when both of us are deceased, the deed will transfer to our daughter.

Just sent a Simple Estate Affidavit for my deceased mother. She has lived in Portland, Oregon, since 1965 and returned to Canada in 2016 due to dementia. I couldn’t change the place of death, but the death certificate is Canadian. I'm trying to access her bank account and safety deposit box. Looking for input on the form and whether it’s correct. Hi, my husband passed away last Sunday and the bank is requesting a new certificate of trust since the last one was in 2001. We have a living trust and I filled in the grantor with myself and my husband as grantors as stated in our trust. Do I just need to have it notarized and take it back to the bank, or do I need to take it to my attorney? Thanks for your help.

I am just unsure about the preparer and return-to section. Also, my father says that there is a lien on the house through the student loans he took out to help me get through school. [OR] What should I do if my mediation eviction order was blank and I want to extend it?

None of those portions of the agreement were filled out by the mediator. The copy of the agreement I received is blank. Is my best course of action to request the original filed copy from the court clerk? Shouldn’t those sections contain some information? I didn’t realize they were blank when I signed.
